							<p style="caret-color: #000000; color: #000000; font-style: normal;">Shoulder pain during bench press is one of the most common issues in the gym.</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">If you feel a sharp or aching pain at the front of your shoulder when pressing, you’re not alone. The good news? Most cases aren’t serious &#8211; and you don’t need to stop bench pressing completely.</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">The classic example is someone feels a sharp pain at the front of the shoulder &#8211; worst at the bottom of the press. Other movements like rows feel fine.</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">It often feels like and labelled as an impingement, rotator cuff tear or bursitis.</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">It sounds mental, but impingement actually happens in everyone! Every time you elevate your arm there is some form of impingement happening in the shoulder. Whether or not it is painful often depends on how sensitive the structures in the shoulder are. Therefore a better way to look at it: Load &gt; Capacity causing sensitivity.</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">Load = what you’re doing/have been doing</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">Capacity = what your shoulder can tolerate.</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">The goal in rehab &#8211; slowly build up load so slowly build up your capacity.</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">So does this mean stop bench pressing? Not necessarily. The beauty of the bench press is we can change so many factors to make it more comfortable again.</p>
<ul style="caret-color: #000000; color: #000000; font-style: normal;">
<li>
<p>Lighter weights</p>
</li>
<li>
<p>Limited range of motion</p>
</li>
<li>
<p>Isometrics</p>
</li>
<li>
<p>Smith machine instead of dumbbells/barbells</p>
</li>
<li>
<p>Reduced sets/reps/frequency.</p>
</li>
<li>
<p>Elbows more narrow vs elbows more wide.</p>
</li>
<li>
<p>Wide grip vs narrow grip.</p>
</li>
</ul>
<p style="caret-color: #000000; color: #000000; font-style: normal;">Any variation that is tolerable will do. Your goal is to just gradually progress the above towards the type of bench press you are wanting to get back to.</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">If even just the movement unweighted hurts, then maybe we need to give the bench press a small break temporarily.</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">In this time, we make every other movement around the shoulder strong. Particularly internal and external rotation, as well as shoulder flexion and abduction. What does this mean?</p>
<p style="caret-color: #000000; color: #000000; font-style: normal;">Apply the above factors to a:</p>
<ul style="caret-color: #000000; color: #000000; font-style: normal;">
<li>Cable shoulder external and internal rotation</li>
<li>Front raise</li>
<li>Lateral raise</li>
</ul>
<p style="caret-color: #000000; color: #000000; font-style: normal;">Now we have a complete shoulder program keeping you generally strong. As you improve, we inch closer and closer to that 100kg bench press you have been working on!</p>						</div>

							<p>I see a lot of players from our local soccer/football/netball leagues coming in with a vast array of injuries. What’s common between them is the same question &#8211; “How long until I am back?”</p>
<p>From experience, it is a tricky question to answer. There are many variables that come into play, therefore accurate estimates of a return to sport time are difficult. Someone may have a higher baseline in strength or plyometric ability. Another may not. Someone may have a very positive outlook with returning. Another may not. The variables are endless.</p>
<p>We can use our assessments and what the research says to estimate the time frame and it may give us a guide. But the most important factor is not time alone &#8211; it’s probably your rehab.</p>
<p>A scenario:</p>
<p>A senior football player, lets call him Jack, comes to see me with a hamstring strain from last weekend. It’s a decent one &#8211; grade 2. We do our assessment and see a severe reduction in his strength, endurance and range of motion. We give him some initial exercises to get started with &#8211; perhaps the Askling exercises.</p>
<p>Jack is eager to get back &#8211; the pressures of performing for himself and the team, the loss of income due to not playing and of course the mental side of things, missing out on the sport he loves.</p>
<p>Jack asks me &#8211; “so how long until I’m back”</p>
<p>I respond with &#8211; “based off your measures and the research for grade 2 strains, you are likely 6 weeks away from returning”</p>
<p>Now that all sounds good, but this can go two ways in Jack’s mind.</p>
<ol>
<li>“Okay, so in these 6 weeks I need to rebuild my capacity through progressive exercise and loading, then I might be ready to return to sport pending what my assessment measures are like” &#8211; the correct option.</li>
<li>“Sweet, if I just do these same exercises for 6 weeks I will be all good to return to playing at exactly 6 weeks” &#8211; the incorrect option</li>
</ol>
<p>Time alone is not a rehab plan. This mindset will likely result in reoccurrence of the injury due to not rebuilding capacity then resulting in more time lost due to injury.</p>
<p>There are steps to a rehab plan that bridge the gap from the injury to performing at your top level.</p>
<p>From basic exercises to desensitise the area and increase range of motion, to starting some strength training progressing to very heavy loads, and lastly returning to plyometric and power drills such as hopping, sprinting and jumping. NOT &#8211; do these 3 exercises for the entirety of a 6 week period to return saying “she’ll be right”.</p>						</div>

The common advice &#8211; “you’ll be right, it’s just an ankle sprain”.</p><p style="caret-color: #000000; color: #000000; font-style: normal;">The primary reason we can’t call it “just an ankle sprain” is due to the incredibly high recurrence of them. After your first ankle sprain, 40% of people go on to develop chronic ankle instability (CAI) &#8211; a condition that results in more time away from sport and impairs performance significantly.</p><p style="caret-color: #000000; color: #000000; font-style: normal;">Upon testing we often see severe reductions in balance, proprioception and often reliance on vision and attention to compensate for an impaired ankle. This is why resting until it “feels better” does not cut it.</p><p style="caret-color: #000000; color: #000000; font-style: normal;">Early rehab focusing on strength, balance, and movement retraining is key. The goal isn’t just to get rid of pain &#8211; it’s to rebuild confidence and control in the ankle so it can handle the unpredictable nature of sport.</p><p style="caret-color: #000000; color: #000000; font-style: normal;">So, next time you or someone you know rolls an ankle, don’t brush it off. Treat it with the same intent you’d give any other injury. Because if you don’t rehab it right the first time, you’ll likely be dealing with it again… and again.</p><p style="caret-color: #000000; color: #000000; font-style: normal;">At Goulburn Valley Physiotherapy Centre, ankle sprain management is one of our bread-and-butter conditions. 							<p>After working with people with sports injuries in Shepparton and Numurkah for nearly twenty years, I have seen a lot of different approaches to rehabilitation and recovery. The way an injury is managed can have profound implications on the rate of recovery, the level of recovery, and the rate of reinjury. Some people make full and sustained recoveries, while others seem to have a permanent spot on the injury list. Some may even never return to sport. Here is an outline of “the good,” “the bad” and “the ugly” approaches I have seen.</p>
<p><strong>The Good</strong></p>
<p>The best approach to managing sports injuries has three main components:</p>
<ol>
<li>Good early management.</li>
</ol>
<p>This may involve withdrawal from play to protect the area from further injury, pain relieving modalities such as ice, and professional assessment to determine the nature of the injury.</p>
<ol start="2">
<li>Specific rehabilitation.</li>
</ol>
<p>A specific rehabilitation plan should be developed for each person. This takes into consideration the person’s injury, any impairments and the person’s return to sport goals. For example, despite both players having a hamstring strain, a tennis player’s rehabilitation would look very different to that of a football player as the sports have different physical demands. Obviously, to develop an appropriate rehabilitation plan, thorough and ongoing assessment is required.</p>
<ol start="3">
<li>Addressing of injury risk factors</li>
</ol>
<p>Unfortunately, pre-existing injury is a risk factor for future injury. Each player should be assessed for physical, behavioural and environmental factors that may predispose to future injury. Was there a sudden change in physical load? Are there any strength or balance deficits that need addressing? Are there any lifestyle factors that need to be considered?</p>
<p>Good physiotherapists try to do themselves out of a job. If rehabilitation is done well, recovery is sustained and players minimise visits to the clinic.</p>
<p><strong>The Bad</strong></p>
<p>Many myths have evolved in the space of injury management. People have described many questionable practices either self-inflicted or inflicted on them by others. I’ve heard of:</p>
<ul>
<li>rubbing methylated spirits into an injury,<span class="Apple-converted-space"> </span></li>
<li>drinking vinegar,<span class="Apple-converted-space"> </span></li>
<li>having muscles “put into place,” and even<span class="Apple-converted-space"> </span></li>
<li>having ligaments “untwisted.”</li>
</ul>
<p>At best, these practices are ineffective. At worst they can increase the inflammatory response to an acute injury or delay appropriate treatment and recovery. If a person suggests any of these practices, use your judgment to form your own opinion as to whether they are appropriately trained in injury management.</p>
<p><strong>The Ugly</strong></p>
<p>Now to the worst approaches. Here are a couple of examples that I have seen in the clinic.</p>
<ol>
<li>Ignoring an injury</li>
</ol>
<p>Ignoring something doesn’t necessarily make it go away. I’ve seen examples of sports people attempting play on after knee injury, only to discover ACL and cartilage damage in later assessments. If the area was protected at an earlier stage, the severity of the injury could have been limited, reducing the long-term negative consequences.</p>
<ol start="2">
<li>Alcohol</li>
</ol>
<p>Unfortunately, alcohol and early recovery do not mix. Alcohol may affect the inflammatory process that occurs following an injury. Numerous people have presented to me with swollen and sore joints after a night on the town whilst they were injured.</p>
<ol start="3">
<li>Premature return to sport</li>
</ol>
<p>Recovery takes time. As I have mentioned, injury is the biggest predictor of re-injury, and a premature return to sport elevates the risk further. Often, we are working against deadlines for events, or players are missing out on payments whilst they are on the injury list. People that do well take the view that their body is their priority. A patient approach to rehabilitation will often mean less time away from the sport, or an improvement in performance upon return.</p>

<li>Find a physiotherapist that asks detailed questions about load, training history and context. Not just the location of the pain.</li>
<li>They connect the dots through all aspects rather than just “weak muscles”. They refer to aspects such as physical, mental, technical and lifestyle attributes that influence injuries, performance and recovery.</li>
<li>They give you a plan with a rationale, not just “come in twice a week and we’ll see</li>
<li>RED FLAG: “Your hip flexors are tight, your glutes aren’t firing, and your posture is bad.” That’s lazy, outdated, and meaningless</li>
</ul>
<p style="caret-color: #000000; color: #000000; font-style: normal;">They integrate performance.</p>
<ul style="caret-color: #000000; color: #000000; font-style: normal;">
<li>A good sports physio wants you to return even better than where you once were.</li>
<li>This means your physiotherapist must bridge the gap between rehab and strength and conditioning, addressing aspects such as strength, power, speed and endurance rather than 3&#215;10 of a simple exercise for 8 weeks.</li>
<li>Rule of thumb: A good physio doesn’t stop when pain is gone &#8211; they stop when you can compete at your old level (or higher)</li>
</ul>
<p style="caret-color: #000000; color: #000000; font-style: normal;">They assess, and don’t guess.</p>
<ul style="caret-color: #000000; color: #000000; font-style: normal;">
<li>A good sports physiotherapist reviews aspects such as strength, power, speed and endurance, as well as assesses your tolerance to sports specific movements constantly.</li>
<li>Exercise progressions are based on data and assessments, not just time.</li>
<li>There should be no guess work, you should be able to see HOW you are getting better.</li>
<li>Rule of thumb: If you are not being tested with a wide range of physical tests, your physio is guessing.</li>
</ul>

							<p><span style="caret-color: #000000; color: #000000;">A common question I get asked is “what is the best exercise to help my back?” That’s a reasonable question, considering that almost all of us will end up with lower back pain at some point in our life. Despite how common it is, no two back problems are exactly the same. On top of that, different exercise approaches have had their time in the sun at different times. We can all be guilty (me included) of jumping on the latest trends as they make appearances on our social media feeds or are spruiked by popular gurus. Let’s put that to one side to see where the best evidence lies. </span></p><p style="font-style: normal; caret-color: #000000; color: #000000;">Let’s start with core exercise. Around the nineties we all became obsessed with strengthening our abs to protect our back. This was reinforced by research that showed that people with long term back pain did not turn on their tummy muscles in the same way people without back pain did1. People started being conscious of bracing their tummy muscles when lifting weights or bending over. Approaches like Pilates really took hold and are still popular today. We did find that we were helping people with core muscle exercises. </p><p style="font-style: normal; caret-color: #000000; color: #000000;">Was there something special about core exercises that helped people’s back pain or was it the fact that we just got people moving? Researchers got onto answering that question. People with back pain were given general exercises that included cardiovascular conditioning and strengthening. Evidence emerged that showed that general exercise helped people with back pain too. There was no significant difference between doing core exercises and doing general exercises. </p><p style="font-style: normal; caret-color: #000000; color: #000000;">So where does this leave us? Basically, it’s horses for courses. You need to consider many things like the type of back pain you have and your goals. Like I said, two backs are never exactly the same. Some backs like to stand and arch, some like to sit and bend and there is everything in between. Your goals will also drive your exercise selection. If your goal is to be able to stand and walk further without your back hurting, there is no point doing exercises that solely involve sitting and lying down. Exercises need to be specific to your problem and your goal. Finally, your preferences need to be considered. Some people do well with structured exercise classes on set days of the week, others do well with a program of exercises designed for them that they can complete in their own time at home. The key is to find someone who will work with you to find what works best for you to achieve your goals. </p><p style="font-style: normal; caret-color: #000000; color: #000000;">If you have new or persistent back pain and want to get on with life, odds are that exercise will help. Physiotherapists are highly trained at assessing your back and your needs and can work with you to design an appropriate rehabilitation program. 							<p dir="ltr"><strong>Why is my Achilles hurting?</strong></p><p dir="ltr">You may notice that in the mornings that the first few steps are quite stiff and painful, you have difficulty walking, or are sore to touch over the tendon. The tendon itself may have also thickened and it may be difficult to perform impact exercise. These can be the first signs of an Achilles tendinopathy. This can occur due to a long-term overuse of the tendon, or a recent increase in training load that lacks sufficient rest.</p><p dir="ltr"> You may have taken a period of time off running or training and then once returning, the pain comes back . This can be frustrating as it feels the pain is lingering longer than it should be, even after resting.</p><p dir="ltr"> If this sounds like something you’re going through, it is important you are thoroughly assessed by a physiotherapist. In the meantime, here are three things you can do to help yourself recover.</p><p dir="ltr"><strong> Manage Your Loads</strong></p><p dir="ltr">A common cause of Achilles tendinopathy is undergoing a fast increase in load, without the necessary rest. This could be as simple as getting back into walking after taking a few months off. You may have previously been able to walk 5km everyday, however after having time off, our bodies need time to adapt to an increase in load again. Therefore we need to gradually build up to achieving this. An easy way to determine your loads is to be guided by your pain; if you recognise that an activity is making you very sore afterwards, you’re probably doing too much.</p><p dir="ltr"><strong> Modifying Activities</strong></p><p dir="ltr">For those with Achilles tendinopathy, certain activities can be more aggravating than others, such as walking, jumping or running. One of the main aims of Achilles tendinopathy rehab is to calm the tendon down. This can be done by changing the way we complete our activities so the tendon is not constantly aggravated. An example of this is still going to footy training, however limiting the amount of jumping or sprinting you complete, just for now.</p><p dir="ltr"><strong> Build Up Strength</strong></p><p dir="ltr">It is important to note that complete rest is not recommended, and can actually be harmful to the tendon and may prolong recovery. Therefore, it is important to start building up your strength in ways that don’t load the tendon too much. Examples of this include doing exercises such as a calf raise with a slow, controlled motion, hydrotherapy and isometric exercises (holding a position). As you slowly build up strength, your physiotherapist will be able to progress your exercises tailored to your goals, such as returning to jumping.</p><p dir="ltr"> Early intervention and management of Achilles tendinopathy can help you get back to everyday life and the things you love. A physiotherapist can assist you by completing an in-depth assessment and tailoring your treatment to your specific symptoms and goals.</p>						</div>

<p>FAI is a very common cause of hip pain in athletes playing sports such as soccer, football (AFL) and ballet.</p>

<p>These sports often require repetitive movements that can exacerbate hip impingement symptoms over time.</p>

<p>It occurs during adolescence where repetitive movements in the end ranges of the hip cause increased bone growth around the ball or socket of the hip. It is important to note however, that 40% of adults have signs of FAI on CT scans but do not have pain (Kim et al., 2015). This indicates that signs of FAI on scans do not need to change in order to get out of pain &#8211; and therefore the presence of bony “deformities” on a scan is merely one piece of the puzzle.</p>

<p>Athletes will often present with a “C-sign” pattern of pain &#8211; where when describing their pain, they place their hand on their hip with the index finger towards the front of the hip and their thumb towards their glutes. It can also present as groin pain and associated tightness. Additionally, they may have issues during deep squatting or rotating their hips.</p>

<p>Over time, FAI when not treated well does have some correlation with early onset hip osteoarthritis (Wyles et al., 2017). This highlights how important physiotherapy can be to manage this condition.</p>

<p>Trochanteric bursitis, or hip bursitis, is a frequent cause of pain to the side of the hip. Put basically bursitis is the inflammation of a bursa which can occur for a variety of different reasons and in all different types of people. But what do I do about the pain?</p>

<p>A common misconception is that the inflamed bursa should not be there and therefore needs to be removed. Bursitis of the hip can be anywhere from mild discomfort to severe and debilitating pain. It is one of the most common hip pain related reasons for referral to orthopaedic surgeons, despite rarely being operated on, with gold standard care being conservative management. Such management can include but is not limited to; physiotherapy, consultation with your GP to discuss appropriate pain management intervention such as anti-inflammatory medication where appropriate and in some cases cortisone injection.</p>

<p>It is also common to have other structures nearby contribute to hip pain, be equally as painful, and sometimes, more painful than the bursitis itself. Structures such as the gluteus medius, and gluteus minimus often contribute to symptoms. The Iliotibial band also has an influence around pain on the outside of the hip. Being physically assessed can help to pin point which structures might be contributing most to your hip pain. But back to that bursa.</p>

<p>So, what is a bursa?</p>

<p>A bursa is a small fluid filled sac that acts as a little cushion for the soft tissues around the lateral aspect of the hip. It helps to lubricate the bony part of the outer hip to allow tendons, particularly the gluteus tendons, to continue to move freely while the body is in motion. We have these bursas all around the body to ensure that tendons and muscles function smoothly and with little irritation caused by interaction with bony areas of the body.</p>

<p>So, why is this hip bursa so sore?</p>

<p>The trochanteric bursa can become inflamed and painful as a result of altered movement patterns, frequent compression through certain postures, or in few cases for no apparent reason at all.</p>

<p>Unfortunately, women are twice as likely to suffer from hip bursitis compared with men due to biomechanical differences that see women generally with a greater femoral angle compared with men. This angle creates additional compressive forces through the outside of the hip. Combine this with hormonal changes experienced around this time, and cases of deconditioning through the lumbopelvic region, women through their 40s to 70s are our most frequent hip bursitis patients.</p>

<p>Men and women later in life who find themselves spending more time sitting throughout the day provide the bursa with a constant compressive force at rest. When combined with a reduction in general fitness and muscle strength through the hip and pelvic region both men and women over the age of 80 susceptible to experiencing an inflamed trochanteric bursa.</p>

<p>But it&#8217;s not just &#8216;middle-aged&#8217; women and our older Australian&#8217;s experiencing this turmoil. Young female athletes make up the third most frequent sufferers of hip bursitis with heavy and frequent training loads combined with ongoing development through the hip and pelvic region making for a painful hip in some instances. Most commonly high load walking and running activities or activities that involve a repetitive use of the hip joint and muscles can contribute to lateral hip pain in young females.</p>

<p>So, over-activity, under-activity, biomechanics and stage of life are strong contributors to the development of bursitis. Some components are controllable while others are not. So how do I manage my hip bursitis?</p>

<p>The &#8216;gold-standard&#8217; is to manage conservatively through means of physiotherapy combined with pain management medications prescribed or recommended by your GP. Your physiotherapist will assess the lumbopelvic region for biomechanical and strength related limitations and use these findings to:</p>

<p>&#8211; develop an appropriate conditioning program</p>

<p>&#8211; educate you on how best to modify or adjust your current daily practices to reduce the ongoing irritations to the lateral hip region</p>

<p>&#8211; provide manual therapies as appropriate to assist in reducing pain.</p>

<p>In some cases where the inflammation and pain are severe, a steroid injection may be used to specifically target the inflamed bursa. This approach together with conservative measures that your physiotherapist will assist you with not only helps to address the current acute bout of pain but provides you with the formula to reduce the likelihood of future occurrences to provide more sustainable and long term management.</p>
